Description
Imuran is GSK's branded azathioprine — a purine synthesis inhibitor used as a steroid-sparing immunosuppressant in dermatology. Prescribed for severe atopic dermatitis, pemphigus, bullous pemphigoid, and other autoimmune skin diseases when long-term immunosuppression is needed.

Indications
- Severe atopic dermatitis
- Pemphigus vulgaris (adjunctive)
- Bullous pemphigoid
- Chronic actinic dermatitis
- Lupus erythematosus
Dosage
1–3 mg/kg/day. Check TPMT levels before starting. Start low and titrate based on blood counts.

Side Effects
- Bone marrow suppression (leukopenia)
- Hepatotoxicity
- GI upset (nausea, vomiting)
- Increased infection risk
- Increased lymphoma risk (long-term)
Warnings
- Check TPMT enzyme levels before starting — deficiency causes severe toxicity
- Regular CBC and LFT monitoring mandatory
- Avoid concurrent allopurinol (increases toxicity 4-fold)
- Contraindicated in pregnancy
- Avoid live vaccines
